Miconduct Report Process
For any send off (Player Red Card, Coach or Team Official Send off) complete the NOCRA Send-off form within 24 hours.
CSL and SCDSL:
Mail ID Cards:
IF THERE IS ANY KIND OF ABUSE TOWARDS A REFEREE, PLAYER, COACH OR SPECTATOR; MASS CONFRONTATION; POLICE CONTACTED; GAME ABANDONED PLEASE CALL THE VICE PRESIDENT, PRESIDENT OR ASSIGNOR, DEFINITELY THE SAME DAY.
Tournaments
CSL and SCDSL:
- ID CARD: Pull the Player or Coach ID Card for Violent Conduct, Assault or Referee Abuse only.
- GAME/MATCH REPORT: On the Match/Game Report (provided by the teams) include the name, time and send-off (on the bottom of the page).
- NOCRA ONLINE FORM: For any send off complete the NOCRA Send-off form within 24 hours.
- USSF REPORT & SUPPLEMENTAL: For Violent Conduct, Referee Abuse or Assault complete a USSF Referee & Supplemental Report within 24 hours (Coach or Team Official send-offs as well). Downloads available below.
- Email the USSF Referee Report & Supplemental and a copy of the Match/Game Report to vice-president@nocra.com
- Mail the Player Card right away to the proper authority (see below).
- The VP will review the report for consistency & accuracy and then email it to the proper authorities.

- On the Game Report (provided by the tournament) include the name, time and send-off. Take a picture of the completed Match or Game Card.
- Record the name(s) and ID #(s) so you can complete your reports. Pull the Player or Coach ID Card for Referee Abuse or Assault only.
- For any send off (player or coach/spectator) complete the NOCRA Send-off form within 24 hours.
- For Violent Conduct, Referee Abuse or Assault: complete a USSF Referee & Supplemental Report within 24 hours (needed for Coach or Team Official send offs as well). Downloads available below.
- Email the USSF Referee Report & Supplemental and a copy of the Game Report to vice-president@nocra.com
- VP will provide Instructions on where to mail the ID Card.
- The VP will review the report for consistency & accuracy and then send it to the proper authorities.

Sending-Off Offenses (Red Card)
- Serious Foul Play (SFP)
- Violent Conduct (VC)
- Spitting at or Biting an Opponent or Other Person (SB)
- Denies Goal By Handling The Ball (DGH)
- Denies Scoring By An Offense (DGF)
- Offensive Insulting Or Abusive Language (AL)
- Receives A Second Caution In Match (2CT)
