Referee Match Reports

For CalSouth sanctioned leagues, tournaments, and competitions use the following Referee and Supplemental Reports.
For SoCal (SCDSL) and US Club Competitions use the US Soccer Referee Supplemental Report.

 MiSconduct Report Process

For any send off (Player Red Card, Coach, or Team Official Send off) complete the NOCRA Send-off Form within 24 hours.

For CSL and SCDSL Leagues:
  1. GAME/MATCH REPORT: On the Match/Game Report (provided by the teams) include the name, time and send-off (on the bottom of the page).
  2. NOCRA ONLINE FORM: For any send off complete the NOCRA Send-off Form within 24 hours.
  3. USSF REPORT & SUPPLEMENTAL: For Violent Conduct, Referee Abuse or Assault complete a USSF Referee & Supplemental Report within 24 hours (Coach or Team Official send-offs as well).  Downloads available below.
  4. Email the USSF Referee Report & Supplemental and a copy of the Match/Game Report to [email protected] 
  5. Mail the Player Card right away to the proper authority (see below).
  6. The VP will review the report for consistency & accuracy and then email it to the proper authorities. 


​IF THERE IS ANY KIND OF ABUSE TOWARDS A REFEREE, PLAYER, COACH, OR SPECTATOR; MASS CONFRONTATION; POLICE CONTACTED; OR GAME ABANDONED - PLEASE CALL THE VICE PRESIDENT, PRESIDENT, OR ASSIGNOR THE SAME DAY.

For Tournaments:
  1. On the Game Report (provided by the tournament) include the name, time and send-off. Take a picture of the completed Match or Game Card.
  2. Record the name(s) and ID #(s) so you can complete your reports. Pull the Player or Coach ID Card for Referee Abuse or Assault only.
  3. For any send off (player or coach/spectator) complete the NOCRA Send-off Form within 24 hours.
  4. For Violent Conduct, Referee Abuse or Assault: complete a USSF Referee & Supplemental Report within 24 hours (needed for Coach or Team Official send offs as well). Downloads available below.
  5. Email the USSF Referee Report & Supplemental and a copy of the Game Report to [email protected].
  6. The NOCRA VP will provide Instructions on where to mail the ID Card.
  7. The NOCRA VP will review the report for consistency & accuracy and then send it to the proper authorities.

  • Unsporting Behavior (UB)
  • Dissent (DT)
  • Persistent Offenses (PO)
  • Delaying the Restart of Play (DR)
  • Fails to Respect Required Distance (FRD)
  • Entering, Re-entering or Deliberately Leaving the Field Without Permission (ERL)​​
Sending-Off Offenses (Red Card)
  • Serious Foul Play (SFP)
  • Violent Conduct (VC)
  • Spitting at or Biting an Opponent or Other Person (SB)
  • Denies Goal By Handling The Ball (DGH)
  • Denies Scoring By An Offense (DGF)
  • Offensive Insulting Or Abusive Language (AL)
  • Receives A Second Caution In Match (2CT)​ ​

Expulsions - Coaches or Spectators

Irresponsible Behavior
  • Dissent, offensive or insulting language, persistent questioning, unsporting behavior.
Referee Abuse
  • A verbal statement or physical act not resulting in bodily contact which implies or threatens physical harm to a referee or the referee’s property or equipment.
Referee Assault
  • An intentional act of physical violence at or upon at referee.
