NOCRA Send-off Report

    Complete the following for any Player RED CARD Send-offs. Use the incident report for any other matters.

    When writing a report include the infraction using the Laws of the Game nomenclature. For example on a Serious Foul Play (SFP), 65 min Red Team Player # lunged into Blue Team Player # with his studs directly into the calf. Red Team Player # was sent off for SFP, challenging an opponent with excessive force in a dangerous manner. Blue Team Player # was taken to the emergency room.  

    Coaches and Team Officials (on the Roster) may be sent-off for the following:
    • Failing to act in a responsible manner (persistent dissent)
    • Referee Abuse
    • Referee Assault

    ​BOTH a USSF Referee and Supplemental Report is required for Violent Conduct, Referee Abuse, or Assault.

    Here are examples:

    57m - Adam Smith #18 with the Blue Angels was sent off for Serious Foul Play.  
    Tackle was from behind cleats to back of ankle with excessive force.

    57m - Sojourn McDougal #07 with the Red Angels was sent off for receiving a second yellow card in the match.
    46m player received a yellow card for holding and stopping a promising attack.  57m she received a second yellow card and send-off for tripping a player in a reckless manner with no opportunity to play the ball.
